The FAA is accepting comments until Nov. 6 on this proposed airworthiness directive (AD), which would require modifying an affected window into a serviceable window. It would also prohibit installing an affected window on any helicopter modified with Supplemental Type Certificate SR00223IB, released Mar. 18, 2024.

The US National Transportation Safety Board (NTSB) has determined the probable cause of the accident to be the flight instructor’s inadequate supervision and delayed remedial action, which resulted in a skid contacting the terrain. Investigators cited the flight instructor’s improper judgment to allow the student pilot to manipulate the flight controls at a low altitude/hover as a contributing factor.
