NTSB preliminary report: MD 369FF, Superior, Arizona, Jan. 2, 2026

The US National Transportation Safety Board (NTSB) report indicates that a witness saw the helicopter strike a highline/slackline before the tail boom separated from the fuselage. Investigators found slackline webbing embedded in the vertical stabilizer. Four people died in the accident.
