VAI Daily Dec. 6, 2024 NTSB final report: Sikorsky UH-60A and MH-60R, San Diego, California, Nov. 22, 2022 The National Transportation Safety Board (NTSB) has determined the probable cause of the midair collision involving two helicopters to be the failure of the MH-60R crew members to maintain visual separation from the UH-60A while complying with the local controller’s instructions to go around. The NTSB cited nighttime conditions and the lack of a safety alert from the controller to either helicopter as contributing factors to the accident.
