USHST offers pilots resources to help ID hazards, boost awareness

The United States Helicopter Safety Team (USHST) is highlighting practical tools already available to help pilots identify hazards and strengthen situational awareness, including the Hughes App (iOS), a free commercial application available through the USHST that includes slackline depictions, geo-referenced helicopter charts, weather and FAA weather cameras, risk-assessment tools, offline chart storage, and mobile access to USHST safety resources.

The US National Transportation Safety Board (NTSB) determined the probable cause of the accident to be the student pilot’s failure to maintain rotor rpm during a simulated autorotation, which allowed for a resonance phenomenon to develop. Contributing to the accident was the flight instructor’s inadequate monitoring of the student pilot’s collective control inputs.

The US National Transportation Safety Board (NTSB) determined the probable cause of the accident to be the maintenance personnel’s noncompliance with maintenance manual procedures that required
the removal of the overrunning clutch output bearing during the regreasing procedure. The omission resulted in inadequate regreasing of the bearing and the subsequent failure of the overrunning clutch assembly.
