VAI Daily Jun. 30, 2026 NTSB final report: Hughes 269A, Gettysburg, Pennsylvania, Feb. 21, 2026 The US National Transportation Safety Board (NTSB) has determined the probable cause of the accident to be the flight instructor’s failure to identify the excess power required to lift off from soft and muddy ground, which resulted in a loss of control and subsequent collision with a hangar during takeoff. The omission resulted in inadequate regreasing of the bearing and the subsequent failure of the overrunning clutch assembly.
