VAI Daily May. 20, 2025 ATSB report: Leonardo AW139, Longford Heliport, Victoria, Aug. 2, 2024 The Australian Transport Safety Bureau (ATSB) has determined that an instructor pilot became spatially disoriented when the helicopter inadvertently entered fog during a winch training exercise, resulting in a ground proximity alert when the helicopter descended below 150 ft. asl. The captain took control of the helicopter from the instructor pilot, initiated a climb, and exited instrument meteorological conditions. VAI Daily May. 20, 2025 NTSB final report: Robinson R22 Beta, Brady, Texas, May 14, 2023 The US National Transportation Safety Board (NTSB) has determined the probable cause of the accident to be the non-certificated pilot’s failure to engage the collective friction control during an on-ground engine run, causing the helicopter to lift off unintentionally. Investigators cited the pilot’s physical and mental conditions, and the use of potentially impairing medications, as contributing factors. VAI Daily May. 20, 2025 Medical crew makes emergency landing in Pennsylvania (VIDEO) (WCAU) – A medical helicopter went down May 19 into the front yard of a residence in Montgomery County. Three crew members escaped injury in the incident.
